On the several club websites that I administer, I'd like to create an email form, sort of like the Contact Us form, but for use by club members who would like to anonymously register a comment, suggestion, concern, or complaint with club officers. Some club members, especially newer members, aren't yet comfortable enough to express themselves openly, even to their mentors. I have reason to believe that unexpressed, and therefore uncorrectable, issues have caused members to leave a club. I want to create a mechanism by which these shy members can express themselves - anonymously - to club officers, so that we can correct the issue before it causes the member to leave the club. Something like the Contact Us form, only with just one or two text entry boxes (one for Comment/Complaint body and maybe a second for Subject), and "laundered" through the website mail server to mask its origin, would seem to be a possible mechanism for this.

Maybe this could be created as a custom web page/form accessible only to members? My HTML is pretty rusty, so I'd need some assistance or least some source code that I could use as a starting point.

P.S. I also plan to initiate availability of a "Comments" box at meetings, but I know that some members are so shy or "paranoid", they will be concerned about being seen putting anything into the box.

Sorry we will not be doing that.

1) we do not support forms

2) if you really need a form there are 3rd party sites you can use.

3) Toastmasters is supposed to be a safe place so why would any member feel that they couldn't speak up about an issue. We will not support anonymous anything.
