Unable to Create new FHT 2.0 site for a club

I am unable to submit the form for creating a new FTH 2.0 site. I am running Windows 8.1 and using the Chrome Browser. The symptoms are that the Submit link is shown in light text but will not fire to submit the form.

We are looking at the problem.

It appears that someone in the past has created the site but it appears that it was never used. Can you add me as an administrator? If that is not possible can you please send me the contact information for the person that is listed as the administrator.
