Also, in addition to what Brian has noted (I agree), you do not have any members entered in Membership Management (MM).

The FTH system cannot fetch member contact info from the TM international website (not allowed to)... it must be entered in MM and officers must be assigned there if you want to set things up by officer such as what you are doing for the contact form. This is the only way it will work.
