When I bring up the 8103E evaluation form on our freetoastthost web site

the evaluation form

has an error. This is a fillable form and when you fill in the Speech Purpose statement box it is erroneously replicated on the second page in the General Comments box. I checked with Toastmasters International and they have a corrected copy of that form on their website.
Please advise if this can be fixed.

Thank you for addressing this. Does the correction take place later?
Today I went to the agenda

and accessed that evaluation form again
accelorators.toastmastersclubs.org/info/...ech_with_Purpose.pdf
and it still seems to have the error that those two entry fields are mirrored.

I am attaching the corrected form that was provided to me by the customer support at Toastmasters International.
