You will get that if the person clicked the Is This SPAM? link in an email to an officer email address or other public address... that is the other way people get blacklisted.
Not unknown at all. From your website... Is this SPAM? links are enabled. 
There were some recent phishing attacks that may have been the root cause of your member clicking the link that resulted in another member getting blacklisted. I have tweaked the logic to try to prevent that from happening in the future... if you are still having issues, just disable the Is this SPAM? links. Regards,
