I was recently inspired to add an Executive Meeting template to our club.
I can create or repurpose one of the existing agenda templates.  but in this mode:  Meeting Agendas form, Agenda Template tab, there is no control presented to add another agenda item to the template I'm editing.   So currently it seems I have edit an existing meeting then save as template to accomplish this.
I don't remember that from before.

Your image didn't work, and you haven't provided your club number.  
If you have agenda edit permissions, you should be able to edit any agenda or template, and save it as either a template or agenda. 
If you don't have edit rights to create/edit NEW agendas, you might experience what you're describing.

Strange - I went to edit / remove your apparently bad image, and could see it.

You're in the Agenda Settings area, so you have access to the templates
Just add the title and details for the next role in the "Role / Agenda Item Title" & Details boxes and check the required options  at the bottom of the form and then Save.   It looks okay to me.  See the 'Add Role' section in this documentation:

Sorry, Pam,... this makes the second time this forum has deleted the information in my profile, including the signature I had previously entered.   You can find me discovering that previously on 28 Apr 2020.  But anyways....   I am the admin for Merrimack Toastmasters, club 508.

The image I had tried to paste, shows me in an agenda template form with one role.   I can edit the one role there, but there is no obvious way to add another.   If I'm in other agenda editing forms there is red text  over a section to Add/Edit Agenda Item.   This seems to be missing from the Agenda Template form.   The template I'm trying to edit is the first Executive Meeting

Is there a legible way to add screen shots?

As per my previous response - add the new roles at the bottom of the agenda, then save.  See my image below
To add an image, click on the attachments button and add files -  navigate to your allowed image file, then insert

Okay, I got it. More subtle that the regular agenda editor.
And how did you get the screen shot to render correctly?

There has been a slight change to the software platform - you now have to click the "Attachments" button before you can add tour screenshot/image
