From a review of our logs, it appears that all of the problems are tied to a single sender, Chris Waclawik. Not sure why yet.

What email agent/software is Chris using?

It looks like there were problems for the SMTP server to connect to send the email, and it kept trying. i have a max # of retries set at 11, so what you got may be some remnant of the retry loop. (not entirely sure though)

I have also made a small change based on a hunch.
