New Members cannot sign into website

I have a member who has tried numerous times to login to our Toastmasters website. I have personally entered him into the system. I have checked for any typos on my part yet he is unable to log into the site. He is entering his email, leaving the password blank and receives a message that he has errors in the form, which indicates his email. When I log into and check in Membership Management I get this message: The following 1 people have NOT YET ACCEPTED
their addition (not yet "opted-in") and will be DELETED SOON:Jack Chrzan (Member), deleted after 2 days

Have Jack send you an email from his gmail account. I strongly suspect that the number behind his name is incorrect in your website.

I have sent him the opt-in email again

Let us know the exact error he receives when he clicks the accept link in the email.
