This is quite possibly the Chrome auto-fill feature. I think that Chrome adds the following icon to the input, also.
(We do not provide that icon.)
Autofill is one of those things that is very inconsistent from one browser to the next and from one platform to the next. A major PITA.
No way to shut it off on a per website basis.
"Maybe" a workaround: I just added the tweak I use for the login dialogs to the speech title box. Hopefully, that will handle it. (but it may not) If it does not, not much else that can be done other than you shutting off the autofill feature which will affect every website you visit w/ that browser.
