Club identifier duplicated in "Contact Us" From field

I've noticed this a couple of times and couldn't find it reported here. When someone visits our site and uses the Contact Us form, I get an email with a From field that looks like the line below. The From field contains two copies of "via Toastmasters Club 2263055". This isn't a big problem.
