Problems adding Contact Name to Contact form

We're having problems adding names to our Contact Us Form.

Despite what appears to be correct settings in the Email & Contact Form module (three names listed), only one name appears to the user( outside the editor module). Any assistance would be appreciated!

Names and phone numbers are only shown if:

1) The checkbox is checked to indicate that a contact can be called.
2) The selected contact has a phone number entered in Membership Management.

Of the 3, selected contacts, only one (Julie) meets both of the above criteria.
