Contact Us Form not adding info to Prospects List

We have had our Contact Us form set up to add requests to the Prospects List. It has not been doing that for a long time. I have attached a screen shot of the configuration page. If anyone can help me understand why this isn't working I would appreciate it greatly.

The prospect will only be added to the list if they check the box at the bottom of the form.  The message received by the contacts will say 

Include in future emails
  about Meetings & Events: YES

at the end (if it says 'NO', they haven't checked the box so won't be added to the list).  
Also, if enquiries from Find-A-Club are being routed to your contacts, they won't be included as they aren't coming directly from your 'contact us' form
