Migrating to FTH4
Your club admin received the instructions to update their admin password to access FTH4. (FTH3 admin passwords will NOT work in FTH4). Follow the instructions in the email and use 'Member sign in' in the top right corner to access the Club Admin account.
If you are not sure who the club admin is, in your member account; look at 'Club Officers and Members' > "Club Email addresses". The LAST item in this list is the person who is currently listed as your club admin.
If this is not correct, please complete and submit this form to change your club admin:

Just to be even more clear, I've included screen shots from the admin module for the Contact Us form and the form as it appears to a guest. The Closing phrase doesn't appear on the form. The result is the same on several browsers on my Mac (Safari, Firefox and Chrome), Windows 10 Explorer and iPad (Safari, Firefox and Chrome).

Ok, you are misunderstanding how this works. 
The closing message appears after you click the Submit button. It shows up at the top of the form in addition to the TI logo.
So... you are thinking it should show on the form... there is no room for that and it has never appeared there anyway.
