Lessons Learned Evaluation Form

This morning I am giving a speech from the "Lessons Learned" elective (PM Level 5). When I printed the evaluation form from our ToastHost site, I noticed that the last category "Topic" on the second page has all 5 options circled. I'm attaching a cleaner copy that doesn't have anything already marked.

I do not think those are circles... they are drop-shadow styling. If you click one of them, they do change to indicate the selection.

Nonetheless, your attachment did not come through. I get a 404 error when I click on the attachment you provided.

We print our evaluation forms off rather than submitting them online, so the circles (aka drop-shadow styling) remains on the page, but it exists only on that last "topic" category and nowhere else on the form. That is, seven out of the 8 categories don't have circles around them. The official evaluation form from TI's Pathways site has no drop-shadow styling on any of the forms. The reason for bringing this up is that it makes the appearance that the speaker has already circled all five numbers for the category of "topic." I hope my 2 attachments of the TI form vs the FTH form came through this time. As always, I truly appreciate all the hard work you and the others do to make life simpler for us.
