I have never received information to set up the Toast Host. Kdraz is no longer involved with the club,

I have updated club information and still have not received information to set up free Toast Host for Arias Toastmaster Club

Curious why would you submit a FTH site request form when the TI contact information is incorrect and you were told while the form was being completed where the authorization email was being sent?
