trying to add new member (again) and name is crossed out

Help!  I'm trying to add a new member and his name is crossed out when I try to send the welcome email.  I added his name, I tried re-sending the opt in email and sent him a note to check.  It's still crossed out.

1.  What am I doing wrong?   This is the 2nd time it's happened. 
2.  How do I fix this?
