Mentor/Mentee Request Form was created by babbler

On the home page in the members section this link "Mentor/Mentee Request Form" is lost. How can I edit this field? Can I get the link back? Assuming it was working in the past.

The mentor form has not been set up look at the admin control panel - Email & Contact Forms

On the email and contact forms tab in the admin console, choose a member to be the contact for the mentor/mentee requests.
