File up loading

Our site has manu files on it from prior admin. Questions
1.How to up load. Read basics and seems staight forward but:
2. Some of the prior files show up and some don't.
3. Some are in file folders and some aren'y

So:
A. What is the basic format / rules for uploading?
b. How do you set up file folders?
C. How do you assign the files to a file folder.
D. Why don't some of the files show up?

To create a folder is really easy... You type the name of folder; whatever you want it to be called, such as if it is for the VP of Membership, you type: VP of Membership: (then upload the folder)

The order is: Upload files 1st, set folder names 2nd.
