There are a couple of things that may have caused you not to receive the Level Completion Request email from Basecamp.
1) Has the member requested the level completion? I am assuming that this is a Level 1 completion. The member needs to go to their transcript and activate the 4th item in their curriculum to signal that they have completed all the tasks
2) Is the member concerned a dual member? If so, and they have completed step 1) above, were they logged in as a member of club 4664? The completion request is sent to the club the member has set as their "home" club.
In the attached screenshot note the Status: message. This will tell them where the request is up to in the process
If the ideas above don't seem to answer your question, try logging in as Base Camp manager and check the Pending Completion Requests tile. The request may be hiding there.

I think the dual membership issue is particularly thorny and is likely to cause a lot of confusion and headaches for clubs moving forward.
Because Base Camp is essentially a corporate learning management system (LMS) that WHQ adopted, having a member of two clubs would be the equivalent of an employee working for two companies... unlikely to be supported in an LMS (an edge case). However, it is much more likely that a member could be a member of two clubs... WHQ's workaround that requires you to set which club you are working with (your "home" club) before using Base Camp
is a bit hacky at best.
