An Error message says that there is a problem with Javascript, that it has to be downloaded. It already was downloaded. I added Java Update 361 but still could not get the download to open.

What folder and what file are you referring to?

I was able to download your 800-Membership_Application.pdf with my MAC and safari.

Try clearing your browser cache and see if that makes a difference.  (Let us know.)
