Contact Us Form not adding info to Prospects List

We have had our Contact Us form set up to add requests to the Prospects List. It has not been doing that for a long time. I have attached a screenshot of the configuration page. If anyone can help me understand why this isn't working I would appreciate it greatly.

I just tested things and they're working properly.  Not being a gambling man, I won't proffer the odds, but it appears that though you have had the Prospects List feature enabled for many moons, nobody has yet checked the "Check this box to be included in emails about future meetings & events" checkbox.  I did so, and my e-mail address showed up on the Prospects list.  The key to this working is found in the FTH documentation at support.toastmastersclubs.org/doc/item/contact-forms under the "Saving information entered in the form" heading in the italicized, parenthesized informaitonal text: (If the person checks the checkbox at the bottom of the form.)
