Vocal Variety Evaluation Form

Hi,
The Evaluation Form for Vocal Variety in every Path appears to be corrupted.  I can open the form but when I try to save it it does not save and the error message states that the file is corrupted.  In addition when I try to enter the assessments on page 3 it only allows one assessment to be entered.  If I select another assessment it clears the previous entry. I also tried to enter the form in Word and I receive the same corrupted file message.

I'm not seeing any "corruption" issues when saving the form.  What operating system/browser are you using?  Have you tried saving the blank form before making any changes?

Yes - the radio buttons seem to be programmed for one response only for the page, rather than one per question.  That is a TI/basecamp form, please report that issue to TI.  Until that is fixed we'll have to fill in a hardcopy.

I saved the form using Firefox, Chrome and Edge.  When trying to open the form in Adobe Acrobat 8 Professional I receive the following message: "There was an error in opening this document.  Acrobat cannot decrypt this document."  I can edit the first two pages of the document using FTH and am able to print the form.  My workaround for the Page 3 issue is to manually edit the printed file and then get the form to the Speaker.

I will inform TI and hopefully they can fix it before the new Base Camp is on line.
