FTH link to Membership Application is broken

Hello Support Team.

I've noticed the FTH link to the Membership Application has been broken for many months.  We had a potential member in our meeting today who wanted to download the application to join the club but could not.  We are club 3717260 - Kenton Place Toastmasters.

This is the error I get when the link  is accessed.

This XML file does not appear to have any style information associated with it. The document tree is shown below.
