Agenda shows different on phone vs computer

1. We had a incident where a member logged on via both computer and phone at the same and the agenda sign-ups were different on the two devices.

2. We also had a recent situation with someone signed up for a role and another person did not see a sign up and signed up for the same role as well--only one person showed up on the printed agenda.
I know I've experienced an issue in past when I logged in some time elapsed then I signed up for something. I discovered someone else had logged in and signed up for another role that did not show up w.hen my screen refreshed.

3. I can see some members have signed up for a role but the Access Log shows they last signed in 17 days ago--I know they just signed up in the last day or two--why is the latest log in not showing in the Access Log?
