Develop Your Vision - needs second presentation and evaluation

Hi, the project Develop Your Vision requires two presentations and two evaluations. Please add to the drop-down when selecting a project using the agenda.

Is there a 2nd evaluation form that you can provide to us? (just post here as an attachment)

The one from TM contains both.

I separated it and they are attached, but I do not know how to retain the interactive feature and attached the master too.
