Brian, three evaluation forms you requested

Hello Brian,

We are running our meetings online and are using a custom, paid for, form for evaluation feedback.

We have used the standard TMI evaluation form as a model. A combobox with the names of all members allows us to fill the form in online, and send it to the speaker upon completion at a push of a button rather than as an attachment. Is there something similar in FreeToastHost to this? If not, might this be added?
