Contact Form Considerations

My club is fortunate in that we have many active members and a steady stream of guests. We noticed that some guests join, stay for only a short while, and then stop attending. And we noticed that such members have some characteristics in common. For example, members who live in the neighborhood are the most likely to attend on a regular basis, and our club does best in terms of retention when there is a lot of diversity of occupations within our membership. With this in mind, we hope to pose some questions to interested parties in order to assess their candidacy before inviting them to a meeting.
  1. Can additional fields be added to the contact form?
  2. Alternately, can an auto-response be established so that we could direct prospects to a form of our own design?
  3. Or, alternatively, who are the default recipients of messages to the contact-####### email address - perhaps it would make sense to produce auto-responses from one of these email addresses?
  4. Any other ideas about how to achieve this?

No, a contact form is exactly that. A form to make contact with.

The contact email goes to 3 club officers as selected by the admin. 

Remember that auto reply emails can be a form of spam.

Unless your club has stated membership requirements, you should not be filtering who can and who cannot join your club.

If you do not like the built-in contact us form, you are always welcome to create your own form with something like Google Forms.  You can just shut off the internal form then.
