Prospects list - not showing inquiries who ask to be added

We've had a lot of recent use of the fabulous CONTACT US feature. However, the people who check that they want to be included in future mailings are not being added to the prospect lists.
I've attached our settings page.
Please let me know what to change to automatically capture those details - it's a super helpful feature!

Worked for me, I submitted form and I was added to prospect list, they cannot be checking the box at the bottom of the form.
Of course I removed myself.
