Request for new FTH instance for 28678312 J. B. Hunt - SoCal Toastmasters

Dear FTH Support, 

It's unclear to me how to request a new FTH site for a new club and if there's a more correct way, please let me know.
My name is Doug Crocker, Toastmaster Member ID PN-00650481.
I am co-mentoring Club 28678312 J. B. Hunt - SoCal Toastmasters and would like to request a FTH site on behalf of the club's president, LAWRENCE SANCHEZ, who is on copy to this email.  Admin access should be assigned to Lawrence.

The application form for a new site is at www.toastmastersclubs.org/welcome/ (lower half of the page). For an unchartered club, use the Unchartered Club Website tab (the middle one).

Thank you, Jane, for your reply.  Interestingly, the form is not accepting our District number, 100.
 

That's a nuisance. We'll have to wait for Brian to take a look at it. It's definitely beyond my pay grade.
