Recording Permission Form & Upload Worksheet

TM HQ has some guidelines when recording a speech.

When recording a speech there are several steps. I've come up with a form that serves several functions and makes things a bit smoother.

This form helps with these tasks:
  • Gives permission to record the presentation.
  • Helps with titles and descriptions when uploading.
  • Email the speaker a link to the video.

Here is a link to the video release form should you want to check it out.

This is a great form. I would suggest making the statement more broad to include all social media like Facebook (if a club uses) and the club website. It should most likely be mentioned that it will be publicly available for the purpose of promoting the club in the community and anyone may view.

Rob, thanks for your input. Those are some terrific suggestions!

I modified the document to include other web properties. I included another 'this is public' statement to reinforce this awareness. To lessen the freak out factor (FOF), I sandwiched the content with 'only if awesome' verbiage to lighten the tone.

"If your speech is awesome, this video may also be used on other properties like Facebook, Google+, LinkedIn, club websites to promote the club in the community. Anyone may view it but again, only if your speech is awesome."

Again, thanks so much for your feedback. Very much appreciated.
